GLP-1 Troubleshooting

When GLP-1s Don't Work

About 5 percent of women are non-responders. That is the medicine failing you, not you failing the medicine, and there is a real diagnostic path from here.

  1. Talk to your actual doctorNot the online provider who just prescribes the shots. Your real doctor knows your full medical history. Get comprehensive labs done: thyroid panel, cortisol level, full hormone panel, insulin levels, A1C. Find out what else might be going on.
  2. Consider switching medicationsIf semaglutide isn't working, maybe tirzepatide will. If GLP-1s aren't cutting it, maybe you need a different class of medication entirely, or a combination approach. There are options: metformin, naltrexone/bupropion, and topiramate, among others.
  3. Don't give up on yourselfJust because one medication doesn't work doesn't mean you're doomed. It means you need a different approach. Maybe that's bariatric surgery, or working with an endocrinologist who specializes in complex metabolic disorders.
  4. Keep doing the other workEven if the medicine isn't delivering the weight loss you hoped for, the therapy, the movement, the protein, the fiber, the happiness chemicals, all of that still matters for your overall health.

Why it might not be working

  1. Hormones playing a different gameThyroid issues that haven't been diagnosed, PCOS that needs additional treatment, or cortisol levels through the roof from chronic stress. GLP-1s are powerful, but they're not a cure-all for every hormonal imbalance.
  2. Other medications working against youSome medications, like antidepressants, antipsychotics, or steroids, can make weight loss nearly impossible even with GLP-1s on board. This doesn't mean you stop taking your necessary medications; it means you need a more comprehensive treatment plan.
  3. Metabolic adaptationIf you've been yo-yo dieting for decades, your metabolism might be so adapted to restriction that it's holding on to every calorie for dear life. Your body isn't broken, it's just really, really good at survival.
  4. Undiagnosed insulin resistance or type 2 diabetesSometimes the dose or type of GLP-1 isn't strong enough for the level of metabolic dysfunction in your body. You might need a different medication combination or a higher therapeutic dose than what's standard for weight loss.
  5. The root cause is not metabolicIf you're still in a toxic relationship, working a soul-crushing job, dealing with untreated trauma, not sleeping, or chronically stressed, your body is going to hold onto weight as a protective mechanism. The medicine can't override survival mode.

And if it still does not work

If this medicine doesn't work for you, you are not a failure. The medicine just wasn't the right tool for your specific situation. That's it. That's all it means. Health isn't about weight loss. It's about how you feel in your body, how you move through the world, how you treat yourself, and how you let others treat you. There are other paths. This isn't the end of the road. Don't give up. Just pivot.
